The Cuba d'Viajeros Travel Form is a single form replacing the three physical forms required previously – International Boarding and Disembarkation Card, Customs Declaration Form, and Traveler's Health Affidavit.
The Travel Form is mandatory for all travelers entering Cuba by air, whether Cuban or foreign. Since January 1, 2022, only online forms can be used.
Air passengers must complete the form within a week before each visit to the Republic of Cuba. It is simple: provide the requested data in the form, pay the fee, and receive the approval via email.
Before boarding the aircraft, travelers will need to present the Cuba d'Viajeros Travel Form to Cuba’s airline staff. The Sanitary and Customs authorities will also require the QR code when entering the country.

IMPORTANT: The Cuba d'Viajeros Travel Form is not a visa. If your country of nationality is not exempted, check if you are eligible for a Cuba eVisa for an easier application.

The airline operating in Cuba will check if you have a digital or paper copy of the form before boarding the flight. If you are traveling as a family, each family member traveling to Cuba must complete the form.
